Social Prescribing

Working alongside your GP to improve your health and well-being

Available in:Darlington
  • Individuals can be referred by any health professional (GP, Nurse, Health Visitor)
  • Led by Darlington Borough Council and delivered in partnership with voluntary organisations
  • Aims to maximise quality of life with non-medical sources of help in their area
  • Offers on-going support and signposting to improve wellbeing

About the Social Prescribing Service

The Social Prescribing service is led by Darlington Borough Council in collaboration with four key local voluntary organisations:

Age UK North Yorkshire & Darlington
700 Club
Darlington Mind
Darlington Association on Disability

The service operates through referral via health professionals including GP's, Community Psychiatric Nurses, Health Visitors & Social Care Professionals.

Through agreement with the individual, their information will be securely shared with each of the partner organisations who are equipped to recommend a course of action to improve their overall heatlh and wellbeing.

This includes:

  • Information about services available in Darlington that may be beneficial or useful to improving Health and Wellbeing
  • More intensive mentoring to support and take advantage of activities and services for the first time, including support to do this.

The supporting organisation will then follow-up with the individual after one month and three months to see how the support has benefited them. This information is then fed back to the GP/Social Worker and assessed on how positive an impact it has had.
